The Online Forms for Tracking Gift Card Allocations:

Please make a single form submission for each major Recipient.
IE: List School Boards not schools if appropriate.
Download the Worksheet from Below and send it in by Email
IF you have not entered all of your Recipient Donations - GoTo Google Form , Please make a submission for each group of recipients.
Some reminders when entering your Submissions:
Your Name
The Recipient (this maybe a group of Recipients)
Categories: School, Agencies, Women's shelters , Food Programs etc..
Donation Type: Supplies, backpacks, computers...
Stores: Please assign a Stores to each Submission.
Donation Funds: Report Amount for each Submission
You will be able to review your submissions on your Divisional Dashboard. Your LG can forward this link to you.
